1. To register your child fill out the form. Please do not forget to write the school
    your child attends.
  2. Take a copy of the form and send it to my mail (, and
    provide a copy to your respective school with the registration form.
    Fill out the form for automatic payments, which will be made at the beginning
    of the month.
  3. It will be necessary to let the instructor know the previous month in case your child
    has to be absent, for the month to not process the payment. If the instructor is not
    informed your payment maybe process and there will be no refund.
